To ensure care is accessible, I offer two straightforward pathways to secure services.

Pathway A

Medicaid Waivers

State-Funded

If you qualify for North Dakota state-funded programs (such as the SPED, Ex-SPED, or Home and Community-Based Services Waiver), your authorized hours are fully covered by the state.

  • The Process: Request Marie Florvil as your designated Individual QSP through your county human service zone case manager.
  • Rates: Billed directly to the state based on your authorized service plan hours.
Pathway B

Private Pay Packages

Out-of-Pocket

If you do not qualify for state assistance, are waiting for waiver approval, or simply require additional care hours beyond what the state has authorized, private pay is available.

  • The Process: No state assessments or case managers required. We establish a direct service agreement immediately.
  • Rates & Flexibility: Customized weekly scheduling tailored to your family's budget and care needs. 24/7 availability depending on schedule openings.
